We also follow a thorough verification review to confirm that you have made this request so that the risk of abuse or fraud is reduced for everyone's safety. The application will cost a non-refundable C$25 annual membership fee.
This review will take you about 5 minutes.
Your application form will be processed within 3-5 business days. You will then be eligible to participate in select services of Iasis.
Before Beginning
Here's what you will need to have ready...
- Family physician name, address, phone and fax
- Your preferred pharmacy name and phone number
- Your VISA, MASTERCARD credit card information
- Internet Explorer 4.0 or better required*
